Therapist, Vocational Rehabilitation Counselor: 3677 Central Ave., Fort Myers
This position requires a Master’s degree in a mental health related field with experience with the developmentally disabled or persons with severe and persistent mental illness. LMHC, LCSW, or LFMC preferred. This person will serve as the FACT team’s employment specialist, preferably with a master’s degree in rehabilitation counseling. Position Summary: Responsibilities include assessing, planning, developing, coordinating and providing treatment, rehabilitation and support services and work-related service to program individuals with severe and persistent mental illness.

Responsibilities include a variety of skilled treatment intervention services and employment related services.

Clinician IV, Therapist South Ft. Myers – FACT Team: 3677 Central Ave., Fort Myers 
This is a Florida licensed clinician (LCSW, LMHC, LFMT), with documented training and two year’s experience in assessment and treatment of persons with substance use and co-occurring disorders. Position Summary: Responsible for providing all aspects of adult behavioral healthcare services of a FACT Team. Must have clinical skills and experience to assess, plan, develop, coordinate, and provide treatment, rehabilitation and support services to program individuals with severe and persistent mental illness. Will be responsible for a variety of skilled treatment, rehabilitation and support intervention services. Also, serves as the Team’s designated Substance Abuse Services Specialist.

Therapist, Licensed- (North Ft. Myers FACT Team): 939 Pondella Road, N. Ft. Myers: 
This is a Florida licensed clinician (LCSW, LMHC, and LMFT) that is responsible for providing all aspects of adult behavioral healthcare services of a FACT Team. This position requires a Master’s degree in one of the mental health professions and clinical skill and experience to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services to program individual’s with severe and persistent mental illness. This position requires documented training and two years experience in assessment and treatment of persons with substance abuse and co-occurring disorders. Serves as the teams designated substance abuse services specialist. Intermediate level computer skills to include Microsoft Office required.
