POSITION:  Family Stability Case Coach
DEPARTMENT: Social Services & Education
REPORTS TO: Director of Social Services & Education
STATUS: Full-Time, Non-Exempt

Community Cooperative’s school-based Social & Education Resource Center is a new initiative focused on breaking the cycle of poverty in Lee County.  The best way to do this is to create a shorter path to access for children and parents by placing a Family Stability Case Coach in a school setting to offer resources, workshops and one-on-one counseling for issues ranging from job searches and placement, to housing and financial issues, to health and wellness coaching and referrals.

Position Summary:
This position is part of the Social Services & Education Department and will split time between the Community Cooperative offices and a school-based office. Qualified candidates will have strong communication, organizational, interpersonal skills, and knowledge working within a school setting. This position maintains positive, collaborative relationships inside and outside the agency and school while providing support to clients and program staff. The primary duties of the Family Stability Case Coach are to provide assessment, planning and support services to assist program participants to increase financial and housing stability, family management skills, and other self-sufficiency skills.

Essential Job Duties:

·         Develop individualized family/housing plan including identification of existing barriers to self-sufficiency, setting housing and income goals and developing an action
·         Make referrals to appropriate providers
·         Coordinate and advocate for the involvement of appropriate professionals from other agencies
·         Meet with clients on a regularly scheduled basis to discuss, assess, direct and assist in the removal of barriers to self-sufficiency
·         Maintain accurate and on-going documentation in individual client files, record-keeping forms and other reports
·         Follow contract requirements, agency and department policies
·         Provide case management and follow-up services
·         Participate in and contribute to team
·         Attend agency, department and networking meetings as required
·         Create and facilitate educational classes to help clients improve credit scores, increase savings, and reduce debt
·         Other tasks and responsibilities as assigned and required by program guidelines

This is a full-time, benefitted position located in Fort Myers, FL with frequent local travel to nearby agency outreach sites.
Requirements:
  • ·         Bachelor’s degree in Social Work, Counseling, or related field with demonstrated experience
  • ·         Ability to set priorities & manage a complex workload
  • ·         Ability to work and collaborate with a broad array of community partners & team members
  • ·         Ability to work limited and periodic evening or weekend hours for special events
  • ·         Ability to work in a Drug Free Environment
  • ·         Dependable personal transportation to drive to sites
  • ·         Valid Florida Driver’s License
