RENAL TRANSPLANT SOCIAL WORKER - LCSW

We are seeking a compassionate caring partner and patient advocate

1st Shift Part-time- 8:00 a.m. - 5:00 p.m.
Requires flexibility. Will include training on and possibly off-site. Will be key in building the program.

You will perform direct social work services to transplant candidates, transplant patients, living donors, and their families in a multidisciplinary team approach. The social worker is responsible for developing comprehensive bio-psychosocial assessments of candidates. As a member of the transplant evaluation team the social worker is responsible for recommending the suitability of patients for transplantation from a psychosocial standpoint.

Requirements:

Education: Minimum Master's Degree in Social Work
Experience: At least one year social work experience in medical or healthcare setting
License: Licensed Clinical Social Worker required.
Other: Communication and interpersonal skills, basic computer skills, ability to effectively multi-task
